Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Who We Are
TrueSelf Psychological Assessments LLC (“TrueSelf”) provides administrative, scheduling, technology, and payment support for licensed psychologists. TrueSelf is not a healthcare provider. When TrueSelf accesses or processes your health information on behalf of your Provider, it acts as a Business Associate and is required by law to protect your information under the Health Insurance Portability and Accountability Act (HIPAA).
This Notice applies to the licensed providers (Clinical Entities) TrueSelf partners with who are solely responsible for your clinical care and for maintaining the privacy of your health information. Clinical Entities are listed at the end of this document.
Our Legal Duties
We are required by law to:
Keep your protected health information (PHI) private.
Provide you with this Notice describing our legal duties and privacy practices.
Notify you if there is a breach of your unsecured PHI.
Follow the terms of this Notice while it is in effect.
We may change the terms of this Notice at any time. If we do, the updated Notice will apply to all PHI we maintain. You may request a paper or electronic copy of this Notice at any time.
How Your Health Information May Be Used and Shared
1. For Treatment
We may use and disclose your PHI to provide, coordinate, or manage your care. For example, your psychologist may share information with another provider if a referral is needed.
2. For Payment
We may use and disclose your PHI to process your payment for services. For example, we may share limited information with a payment processor (such as Stripe) in order to collect your out-of-pocket payment.
3. For Healthcare Operations
We may use and disclose your PHI for business and administrative purposes, such as maintaining secure records, supporting quality assurance, and using technology platforms like SimplePractice to deliver care.
Other Situations Where We May Use or Disclose Your Information (Without Authorization)
As required by law (such as reporting child abuse or neglect, or responding to lawful requests by public health or law enforcement).
Emergencies where information must be shared to prevent serious harm to you or others.
Judicial or administrative proceedings (in response to a valid court order or subpoena).
Health oversight activities such as audits or inspections by regulators.
Business Associates who perform services on our behalf (e.g., SimplePractice, payment processors, secure communications providers). All Business Associates are required to safeguard your PHI under HIPAA.
For any other uses or disclosures not described in this Notice, we will obtain your written authorization. You may revoke an authorization in writing at any time.
Your Rights Regarding Your Health Information
Right to Access and Copies: You may request to see or obtain a copy of your health information, with limited exceptions. A reasonable fee may apply for copies.
Right to Amend: You may request that we correct or update your health information if you believe it is inaccurate or incomplete.
Right to an Accounting of Disclosures: You may request a list of certain disclosures of your PHI made in the past six years.
Right to Request Restrictions: You may request restrictions on how your PHI is used or shared. While we are not always required to agree, if you pay for services entirely out-of-pocket, you can require that we not share information about that service with a health plan.
Right to Confidential Communications: You may request that we communicate with you in a specific way (e.g., by email only, or at a different address).
Right to a Paper Copy: You may request a paper copy of this Notice at any time.
Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services (HHS). You will not face retaliation for filing a complaint.
Email and Text Communication
If you choose to communicate with your Provider via email or text, please note that these methods are not fully secure and may carry privacy risks. At your Provider’s discretion, your email or text message any and all responses may become part of your medical record. In emergencies, you should not rely on email or text; please call 911.

Vives Psychology, PLLC, a professional limited liability company owned and operated by Dr. Allan Vives, an independently licensed psychologist (“Provider”). Vives Psychology, PLLC is a HIPAA Covered Entity and solely responsible for your clinical care and for maintaining the privacy of your health information.
